We are required by law to prepare a catalog of items donated for the Silent Auction and we need the information requested above to prepare for it.  For that reason, you must return this form by November 29, 2018 to Sandie Prokop, NYFBFAE, P.O. Box 5330, Albany, NY 12205.  Arrangements can be made for delivery of the actual item at a later date.  Thanks for your assistance.

We are pleased to advise you that the New York Farm Bureau Foundation for Agricultural Education, Inc. has been granted 501 (c) (3) status under Internal Revenue Service Code. For that reason, the value of merchandise contributed to the Silent Auction may be eligible for a tax-deduction to the donor.  Please consult your tax advisor as to the tax-deductibility of any charitable donation.
